A CLINICAL DEBRIEFING PROGRAM
The Agency for Healthcare Research and Quality (AHRQ) (2019) defines debriefing as a dialogue between people. In the clinical setting, debriefing creates a platform to discuss and reflect on the thought processes and actions that occurred during a specific clinical event. It is a tool that can be used to learn from any clinical event: when optimal care is provided, when adverse events happen, and when near misses occur. Clinical debriefing can be used as a strategy to improve both individual and team performance. The majority of clinical debriefing models use the plus delta format as the foundation, focusing on two basic questions: what went well and what could be better?
